Medical Information

Stonehill College Sports Medicine department is now utilizing a new online documentation and injury tracking system called ATS. This new system is used for all of our medical records, athlete forms, appointment scheduling, and injury documentation. All of the information needed to complete the process can be found within ATS. It is our hope that this new system will allow for more thorough and efficient transfer of important medical information between student-athletes and the sports medicine staff.

All Stonehill College student-athletes are required to fill out their respective pre-participation medical forms and submit them to the Stonehill College Athletic Training Department using our new ATS online documentation system PRIOR to being allowed to participate in any practice, game or conditioning session with their respective athletic programs.

Below is the link to a PDF containing detailed instructions on how to access the ATS portal and how to fill out all of the required information. Please read and follow the ATS log-in and medical documentation directions carefully (It may help to print off the instructions to have it in front of you as you go through ATS).

AS A REMINDER: All new student-athletes require a physical examination prior to practicing/competing with an NCAA-sanctioned athletic program. This physical examination as per new NCAA guidelines should be within 6 months of first report date. Returning student-athletes do not require a new physical every year, unless there has been an injury or illness that requires new medical clearance to participate in an intercollegiate athletic program.

ALL REQUIRED INFORMATION MUST BE ENTERED INTO ATS PRIOR TO THE DUE DATE:  AUGUST 1.

YOU WILL NOT BE ALLOWED TO PRACTICE OR COMPETE WITH YOUR RESPECTIVE TEAM IF THESE FORMS ARE NOT
COMPLETED AND ENTERED INTO ATS BY THE DEADLINE, REGARDLESS OF WHEN YOUR ATHLETIC SEASON BEGINS. NO EXCEPTIONS!

PLEASE NOTE – The medical forms, documents and information found in ATS are separate from the school’s Health Services Requirements and are used strictly for Varsity Athletic participation only!  If you have any questions regarding your general student medical forms requirements, please contact Health Services at 508-565-1307.

If you have any questions or concerns about the above information and/or any items pertaining to the pre-participation requirements, please do not hesitate to call us at (508) 565-1514.

On behalf of Stonehill College's Sports Medicine Staff, we wish you a healthy and successful upcoming athletic season as we look forward to seeing all our student-athletes in the fall.

Enjoy your summer!
